As of November 1, 2008, Labor & Industries’ Prevailing Wage Department will no longer accept any forms with a revision date older than 05-08. You can find this date in the lower left hand corner of the form. The Prevailing Wage Department will send out of date forms back for correction. They will include a new form for you to complete and send back along with a copy of the incorrect form to verify payment.
